Place of Service List Page Field Definitions

An asterisk (*) following the field name indicates a Required field in the Core SmartCare system. Your system may have been customized to require additional fields.

Place of Service

Select the place of service type from the drop down list.

From

Use the calendar icon to enter the effective date for the place of service.

To

User the calendar icon to enter an effective end date for the place of service. You can leave this field blank.

Priority

Type up to a 3 digit number that identifies the priority of this place of service if there are multiple places that provided service. The Priority number determines which place of service is processed first.

Plan(s)

Identifies any Plans that are specified for this place of service. If the field is blank, then all Plan(s) are available.

Service Area(s)

Identifies any Service Areas that are specified for this place of service. If the field is blank, then all Service Areas are available.

Location(s)

Identifies any Locations that are specified for this place of service. If the field is blank, then all Locations are available.

Degree(s)

Identifies any Degrees that are specified for this place of service. If the field is blank, then all Degrees are available.

Procedure Code(s)

Identifies any Procedure Codes that are specified for this place of service. If the field is blank, then all Procedure Codes are available.

Staff

Identifies any Staff that are specified for this place of service. If the field is blank, then all Staff are available.

Program(s)

Identifies any Programs that are specified for this place of service. If the field is blank, then all Programs are available.
