Service Area(s) |
Identifies any Service Areas that are specified for this place of service. If the field is blank, then all Service Areas are available. |
Location(s) |
Identifies any Locations that are specified for this place of service. If the field is blank, then all Locations are available. |
Degree(s) |
Identifies any Degrees that are specified for this place of service. If the field is blank, then all Degrees are available. |
Procedure Code(s) |
Identifies any Procedure Codes that are specified for this place of service. If the field is blank, then all Procedure Codes are available. |
Staff |
