Identifies the dollar rate charge for the code. You can enter up to 8 characters. Enter a dollar sign and the decimal if desired. |
Start Date |
Identifies the effective start date for the billing code to be used. |
End Date |
Identifies the effective end date for the billing code to be used. |
Insurer |
Identifies which insurer this billing code is used for. |
Unit |
Identifies the number of units for this billing code. For example, enter 60 for 60 minutes. You can enter up to 9 digits. |
