Code/Rate Creation Window Field Definitions

An asterisk (*) following the field name indicates a Required field in the Core SmartCare system. Your system may have been customized to require additional fields.

Field

Description

Billing Code

Identifies the code number assigned to this billing code. You can enter up to 20 characters in this field.

Name

Identifies the name that describes the billing code.

Rate

Identifies the dollar rate charge for the code. You can enter up to 8 characters. Enter a dollar sign and the decimal if desired.

Start Date

Identifies the effective start date for the billing code to be used.

End Date

Identifies the effective end date for the billing code to be used.

Insurer

Identifies which insurer this billing code is used for.

Unit

Identifies the number of units for this billing code. For example, enter 60 for 60 minutes. You can enter up to 9 digits.

<unit measure>

Select the unit of measure for the units using the drop down arrow. Options are:

·        Days

·        Hours

·        Injections

·        Items

·        mg

·        Miles

·        Minutes

·        Trip

·        Units

 

On a claim that uses this code 'From Date' of service always equals 'To Date' of service

When selected, this option identifies that the billing code cannot cover more than one day (or 24 hours) of service.

Hospital Code

Identifies that this billing code is a hospital service code.

Exclude Discharge Day

Identifies that the billing code does not include the discharge date for hospital stays.

List

Delete icon

Click the delete icon to delete a billing code from the system.

BillingCode

Identifies the billing code used to bill the insurance or plan payer.

Name

Identifies the descriptive name of the code.

Rate

Identifies the dollar amount charged for the code.

StartDate

Identifies the effective start date for the code.

EndDate

Identifies the effective end date for the code.

Insurer

Identifies the insurer that the code is used to bill.

Unit

Identifies the unit of measure of the code.
